Had the same problem with my first Springfield 1911 V-16, and it got to the point that my firing pin hole got clogged up with brass shavings. Took a real close look at the gun, and damned if it turned out if the breech face had been poorly machined and was slighly concave. As an aside, the interior of the gun had some big machining marks in the slide, so somebody forgot to change out a cutterhead(s) on a CNC machine soon enough. Sent the gun in with a complaint letter, and got the same gun back, and the breechface was flat, because they had just machined about 20 mils off the breech face!!

Great! Now I got a gun with a 20 mil headspace problem, and I'm shooting 45 Super at that. Sent back in the gun again with a smoking hot letter asking them
WTF??? and got a brand new gun back that was pretty much flawless, and the problem went away. Now my 45 Super brass has flat primers, but no shaved brass from the firing indent. Check your breechface VEERRRY carefully!
