Did the holster cause the gun to fire? NO. This guy was trying to go a lot faster than his basic safety skills (or lack thereof) enabled him to.

Personally I think that some of these holsters with the thumb release makes it so that you are essentially gripping the gun with your three lower fingers when you are going real fast, since your trigger finger should be indexed, and your thumb hasn't wrapped around your grip yet...Therefore suckering people into using the trigger finger to help hold the gun...ON THE TRIGGER. This is due to them trying to go to fast before they get the mechanism down.

Anyway, if you want to quick draw, learn your equipment, or get a different holster.

He offered no advice on how to not have it happen again.

The usual advice is to "be more careful", which is worthless. So he's a step up from most people in this kind of incident.

Safety isn't a matter of care or attention, it's a matter of process. You need to develop processes for firearm handling that maintain safety, and practice them until they become routine.

His process had a flaw. He needs to examine the process, and eliminate the flaw.

We have an inveterate dislike of the profusion of safety devices with which all automatic pistols are regularly equipped. We believe them to be the cause of more accidents than anything else. There are too many instances on record of men being shot by accident either because the safety-catch was in the firing position when it ought not to have been or because it was in the safe position when that was the last thing to be desired. It is better, we think, to make the pistol permanently "unsafe" and then to devise such methods of handling it that there will be no accidents.

Depressing the release with your trigger finger while drawing is a horrible design idea and directly leads to issues like this... Then again this point is probably lost on you and I'm wasting my time. I might as well get back to my project of designing rat poison that looks like real food.

Have you ever used such a holster?

I use a serpa, and it has a index finger lock. I actually think it assists your finger indexing on the frame if you use it correctly. You can certainly miss use it just like any other holster when you try and draw quicker than you are able.

Maybe we should "recall" the 1911 because it has those damn safety's that can get people killed. Or maybe we should recall those damn glocks since they don't have the safety's that get people killed. I mean, it CAN'T be the user's fault for not correctly using their equipment, right?

Where there's smoke stands someone who wasn't careful with their matches.

You could make the argument that nobody should carry with one in the chamber since EVERY SINGLE ND has happened when there has been a round left in the chamber. Isn't that enough smoke to locate a fire? Small guns have been linked to problems, is that a reason to get rid of them, or to acknowledge that such equipment requires more training?

While I acknowledge that the index lock mechanism might make bad draw skills worse (especially if your disengaging the safety before drawing? see 1:30 on in the video), it is still inherently not the holster's fault. The holster is not making the gun go off. I just don't believe we should automatically blame the equipment without the equipment mechanically failing during correct usage...
