At this moment, I cannot think of one.

Not a TV show but the movie Act of Valor is the most realistic action movie you will find. The actors are SEALs and were in charge of choreographing the scenes to make them realistic and tactically accurate. The story is meh, acting is terrible (because they are warriors not actors), but the fighting is awesome.

But to answer your question, I learned a ton as a kid watching Chuck Connors and his modified Winchester 1892 in the Rifleman! It was plenty accurate, I had a bb gun that I modified to do that!

2. Then, for the first time in any show ever, the bad guy decides that the most important thing to do after getting shot at is to pick up the spent/ejected case. And examine it closely. Or maybe not so closely, but somehow it is blindingly obvious that this brass had been reloaded. Ugh. I didn't know reloaded ammo was somehow dramatically and noticeably different from a factory round "Look at THAT crimp!". Plus, why should anyone even think about this? There are weapons all over this show's 'world'. Tara obviously found the gun (remember, Negan's crew previously confiscated Alexandria's arsenal), so why is it not likely it had ammo in it? But just because she DID actually have a reload he is able to figure it out. And then, when he asks who reloaded it instead of just saying "who knows, I found the ammo somewhere and just used it" everyone starts confessing about it so that finally the guy who actually did it gets taken away.

You have to remember back to where that brass came from...when Rosita picked it up it was fired from a Desert Eagle. Therefore, the case head was stamped .357 Magnum as was visible in the scene. Seeing as how it was later shot out of a Beretta would make it suspicious that it had been reloaded. Not that I would know though; I don't reload, kill zombies, or even watch the show. ;)

I once saw a guy get blown clear through a wall after getting shot with a 12-gauge. I was impressed. And I have learned from TV that every type of gun makes all kinds of clicking noises whenever they get pointed at someone.

Many of those directed by John Milius. I particularly like "The Wind and the Lion" but there are several other movies.

I suppose with a consistent cheek weld that is the exact same every time one could theoretically be accurate with the front sight only.

Sure, and when you shoot an AR like it's an M1A with your right elbow way up in the air, not to mention having your nose all the way on the right edge of the charging handle, why wouldn't you have a consistent cheek weld?

Let's say you were offered a great job on set of a big tv show or movie like this, that uses a lot of prop guns that look real, but you were a hardcore 2A guy and always carried where ever you went. I'm going to assume that the producers would not want ANYONE carrying a gun with live ammo anywhere on set for safety reasons.

Would you carry?
Would you quit?

I worked on a set and I didn't carry. No live ammo anywhere on set and nobody allowed to touch firearms that wasn't involved in a scene. Between takes I held on to them and administrative checks were done each and every time one was given to an actor or taken from an actor.

Theyve had a number of gun faux pauxs on TWD.
I forget all the actors names, but there was a scene where one of the Alexandria originals pulled a glock on Rick and we hear a very audible hammer cock.
Also- when the blonde (the governors girlfriend) kills her self with a revolver, we hear the spent case hit the floor.
And in the last episode, Michone has a gun to the head of the girl driving the car. The hammer is cocked and the safety is off on the Beretta. When Michone gets serious asking questions and insists on the answer, we hear the gun cock again as the camera changes angles.

Forget about all the endless ammo mags they have, like so many Hollywood shows/movies. But they are better than most.

How about the 99.85% successful head shot kill percentage that every character has with any type of weapon at any distance?
