Gun in the cockpit

these are pics of the usairways a319 on a flight from denver to charlotte. the captain had his gun out durning flight and at 11,000 feet on decent he decided to put it away. when he put it in it's holster it went off shooting a hole in the side of the airplane. it landed safely at charlotte.

So, this would be the result of the Pilot had an AD when trying to secure his weapon in that outrageous contraption they called a holster?

It was probably pretty tough for the air traffic controller. They were probably yelling at ATC and couldn’t hear a thing.

Nothing goes in the trigger guard of a loaded gun except a finger when firing. Was that holster designed to be used as a security item, much like a trigger lock, but is suppose to be used with and EMPTY firearm?

I'm more embarrased for the holster maker. I can't believe someone that knows about guns thought this was actually a good idea (assuming it's not photochopped).
