Secondly T-Mobile has nothing to do with "WebSense" (now called Forcepoint) from what I can tell. Do you use your phone for work? Your employer may have forced installation of it on your phone.

T-Mobile representative did acknowledge it's their thing after I read her the message on the screen.
If it was employer-related, I would know because I attend my employer IT meetings. My wife's phone does not have anything employer-related, yet it exhibited the same behavior.
I agree that passwords on this site not being encrypted is a terrible security problem.
