Permit to Purchase question

Hello, I had a question on page 2 of the permit to purchase form it has a part that says "The information is to be released to the listed law enforcment agency" I am not sure what to put below here where it has; name, address and contact person. Any help will be much appreciated, thank you in advance.

Alright so this may be a dumb question but for the signature line on the bottom of page 2, is that for me or PD/sheriff? I assume for me but just to double check.

if I remember correctly the second page is the medical release......if so that would require your signature.

When in doubt leave it blank and ask when you turn it in.

Yeah it has stuff for the MN Human Services. Okay I will probably just wait so I don't screw something up.
