OP: Did you fill out anything in #8 or #9? I agree with the "common name issue". Also, half the time that the person calling in the NICS check couldn't understand the operator as they had a thick accent. Make sure the GM rep repeats and verifies anything that he/she may not interpret correctly. As a last question; when filling out the 4473, did you check the right amount of Ys and Ns on question 11?
